About You Before Your First Appointment
Because we want to get to know about you, please complete the Registration Form and bring it with you for your next appointment.
Federal regulations also require that you read and sign the HIPAA Privacy Form prior to treatment.
We've included a few other forms to complete that will be extremely helpful in your treatment. Just print the forms and fill them out before you come in for your appointment.
